Honestly, the root cause isn't fully known

It may be surprising, but ophthalmology does not have one single, complete explanation for why the proteins in the eye's lens break down and clump together. What is well established is a strong set of contributing and risk factors — the conditions that make cataract formation more likely and often faster. Most cataracts are the result of several of these factors acting together over time, rather than one isolated cause.

Age is the single biggest factor

The proteins in your natural lens are largely the same ones you were born with — the lens has very limited ability to replace them. Over decades, ordinary wear, oxidative stress, and exposure to light gradually change these proteins, making the lens progressively less transparent. This is why age-related cataracts are by far the most common type, and why most people will develop some degree of lens clouding if they live long enough.

Environmental and lifestyle factors

  • Ultraviolet light exposure. Years of sun exposure without UV-blocking sunglasses is associated with earlier and faster cataract development.
  • Smoking. Tobacco use is one of the most consistently linked modifiable risk factors for cataracts.
  • Alcohol use. Heavy, long-term alcohol consumption has been associated with increased cataract risk.
  • Poor nutrition. Diets low in antioxidants (found in fruits and vegetables) have been linked to somewhat higher risk, though no supplement has been proven to prevent or reverse cataracts.

Medical conditions and medications

  • Diabetes. High blood sugar over time changes the chemistry inside the lens and is a well-known accelerator of cataract formation, often at a younger age than typical.
  • Long-term steroid use. Oral, and sometimes inhaled or topical, corticosteroids used over extended periods are a recognized cause of a specific cataract pattern.
  • Other eye diseases. Conditions like uveitis (eye inflammation) or high myopia (severe nearsightedness) can be associated with earlier cataract development.

Physical causes

  • Eye injury or trauma. A blow, puncture, or other trauma to the eye can damage the lens directly and cause a "traumatic cataract" — sometimes appearing shortly after the injury, sometimes years later.
  • Previous eye surgery. Certain prior eye procedures, particularly for retinal conditions, can increase the likelihood of cataract formation afterward.
  • Radiation exposure. Radiation therapy involving the head or eyes can contribute to cataract development.

Some cataracts aren't age-related at all

While age-related cataracts are the most common, a small number of cases are present at birth (congenital cataracts) or develop in children and younger adults, often linked to genetics, infections during pregnancy, or metabolic conditions. These follow a different evaluation and treatment path than the typical adult cataract discussed throughout this section.

What this means for you

You cannot control your age, but several of the factors above are within your influence: wearing UV-protective sunglasses, not smoking, managing diabetes carefully, and using steroid medications only as directed can all help slow the pace at which a cataract develops.
